Adult Foster Care (AFC)
Are you looking for a family home setting for care? AFC is designed to provide supportive family living to elderly or disabled individuals who can no longer live alone but would prefer a family setting over an institution. AFC provides assistance with activities of daily living (ADL), nursing oversight, and care management at home with 24 hour supervision. Both participants and caregivers must meet criteria and guidelines set forth by MassHealth.

Group Adult Foster Care (GAFC)
Are you looking for an option that allows you (or your loved one) to remain living independently in your own home? GAFC is designed to support and assist those who are elderly or disabled but able to live on their own. Services are designed to increase participant’s overall health and safety by providing assistance with activities of daily living (ADL), supervision and encouragement with an individual’s personal care needs.

GAFC Family Member HHA/CNA
If you have a family member who currently cares for a MassHealth eligible patient, under the new 2021 COVID guidelines, are now eligible to work for our agency. Please call us at 617.322.9030 for information and the GAFC rules for family employee members.
Eligibility
The eligibility requirements set out by MassHealth are listed below. If you have any questions please feel free to contact an area office near your location.
Adult Foster Care (AFC)
In order to qualify:

be 16 years of age or older
-
your physician must provide a summary confirming the appropriateness of AFC services
-
require daily assistance with at least one Activity of Daily Living (ADL) i.e, bathing, dressing, eating, continence care, ambulation or transfer
-
MassHealth Itss must approve your clinical eligibility
-
be eligible for MassHealth (standard, SCO, or be able to pay privately)
-
have a medical diagnosis and need daily personal care
-
be able to communicate his/her needs (ok to be confused or forgetful)
-
not be harmful to self or others
-
be compliant with plan of care
Group Adult Foster Care (GAFC)
In order to qualify:
-
be 22 years of age or older
-
meet the financial criteria for MassHealth (Standard of CommonHealth)
-
your physician must provide a summary confirming the appropriateness of GAFC services
-
require daily assistance with at least one Activity of Daily Living (ADL) i.e, bathing, dressing, eating, continence care, ambulation or transfer
-
MassHealth LTSS must approve your clinical eligibility
-
reside in identified community housing for individuals who are elderly or disabled OR live in an assisted living residence
-
be financially eligible for GAFC (Financial eligibility for GAFC in an assisted living setting is determined by the Social Security Administration through the Supplemental Security Income Program—Category G (SSI-G))
