MassHealth GAFC Program, Plan of Care, Meal Prep
Under the GAFC program, some clients may be eligible for meal prep during their aides visit. Please call your Area Coordinator or Care Manager to discuss meal prep and your nutritional needs.
Meals-on-Wheels (Nutrition Programs)
Nutrition Programs are available in many Massachusetts city and town locations to residents 60+ (most suggest a donation of $2+). To find a location near you and apply, please click here . If you have been denied by a Nutrition Program, and feel you do qualify and need assistance, please contact your CCH Care Manager.
Menus vary by state, program type, and health condition. (CCH does not have a contract with Mom's Meals), for more information please call their Customer Care team at 1-877-508-6667.

Online Alternative Meal Programs
Meal kits and ready-to-eat programs have become very popular and are all over the internet. Below are a variety of online meal programs at a cost. Programs offer plans, schedules, menus, and special diet options/selections.

Cost: Meals start at $4.29 per person (dietary restrictions)
Cost: Meals start at $7 per serving (choose from two or four servings) and can even be customized for an additional fee; other meal options are available on the site. Delivery is free for orders over $49, and $10 for anything less than that amount.
