Our mission: to provide top-quality care to our clients—seniors and adults with disabilities—helping them achieve the best possible quality of life. We provide personal care services in the home (Adult Foster Care) and assist and support individuals who wish to live independently at home, 7 days a week, 365 days a year (Group Adult Foster Care). AFC and GAFC are MassHealth programs.
We are a person-centered organization. Our key principles are treating people with dignity and respect by being aware of and supporting personal perspectives, values, beliefs and preferences; listening to each other; and working in partnership to design and deliver services.
Our care agents are Registered Nurses, Case Managers, Community Healthcare Workers and CNA/HHA who are committed to our clients. We have on-call services 24 hours a day, 7 days a week, and offer bi-lingual and interpreter services as needed.
Adult Foster Care (AFC)
Group Adult Foster Care (GAFC)
We serve all of Massachusetts. Cities and towns include Boston, SouthCoast - Fall River, New Bedford, Dartmouth; South Shore - Quincy, Weymouth; Taunton, Attleboro; Western Mass - Holyoke, Amherst, Northhampton.
Adult Foster Care (AFC) is a MassHealth program that provides personal care services in the home to elderly or disabled individuals age 16+ who are no longer able to live alone and want to avoid an institutional setting.
Group Adult Foster Care (GAFC) is a MassHealth program that's designed to assist and support elderly or disabled individuals age 22+ who wish to remain living independently in MassHealth appropriate housing.
